Record the reason, old value, new value, person, date and validation evidence for every production change. Use an approved baseline so the complete configuration can be restored, not only the parameter that was remembered.

A one-page change form linked to the configuration checksum is usually sufficient. Define which changes require a full acceptance retest and which need only a focused regression test.
